Here’s How to Get Started

If you’re considering bariatric surgery, know that you’ll be supported at every step. Here are the steps to take to get started:

  1. Take part in an AdventHealth weight-loss surgery information session
  2. Submit your demographic and insurance information and your personal health history questionnaire
  3. You’ll be contacted within three to five business days by your patient advocate to review your insurance and financial information
  4. Attend your initial patient consultation with a provider 
  5. Obtain documentation required by your insurance provider and submit it to your patient advocate
  6. Insurance authorization is obtained
  7. Complete the medical clearances and tests required for surgery
  8. Your patient advocate will contact you to schedule your pre-operative appointment, surgery and first post-operative appointment
